Twisted Testicles Can Be Found Using the Cremasteric Reflex Test The cremasteric reflex is a test to see if the muscle which elevates the testis contracts when the doctor strokes or pinches the medial thigh. Usually this would cause the muscle to contract, raising the testicle. According to the University of California, San Diego in 1994, the reflex was present in all healthy boys in its study above the age of 30 months. With a twisted testicle, the reflex is not present (a 1998 study from the University of Utah School of Medicine, Primary Children's Medical Center, Salt Lake City, Utah found the reflex was a 99 percent accurate test for twisted testicles. ) 7. Surgery Should be Performed as Soon as Possible on a Twisted Testicle Don’t delay because if your symptoms are caused by twisted testicle, because you will normally require surgery and it should be performed as soon as possible. According to a 1996 review study by King's College Hospital, London the testicle was saved in 90 percent of the cases if surgery took place within six hours of the start of symptoms. The testicle was saved in only half of all cases when surgery took place after 12 hours, and in less than 10 percent of all cases after 24 hours of pain. 8. Manual Detorsion Can Help Treat Twisted Testicles Can you untwist twisted testicles? Certainly, you should not try this at home. But, your doctor may be able to untwist your testicles to a certain extent manually. Because time is of the essence when it comes to restoring the blood flow to the testis, manual detorsion – or untwisting – of the testicle can provide a quick, effective and non-invasive treatment. The physician carries out the untwisting procedure, with the patient often under sedation or local anesthesia. If successful, the procedure brings about a dramatic decrease in pain. Surgery following the procedure is still recommended to prevent the testicles twisting again. Manual detorsion isn’t always successful. A 1998 study from the University of Iowa, Iowa City reported that the procedure had only a 26.5 percent success rate, but other researchers (including experts at the Canisius Wilhelmina Hospital, Nijmegen, The Netherlands in 1999) report a higher success rate of up to 80 percent. 9. Both Testicles May Be Treated During Surgery Even if the testicle untwists of its own accord, and the pain goes away, it doesn’t mean the problems of twisted testicles goes away completely. Surgery is needed to prevent the problem happening again, so both testicles are often treated at the same time during initial surgery for one twisted testicle. The non-affected testicle is at risk of testicular torsion in the future, if nothing is done about it. 10. Antioxidant Therapy To Treat Twisted Testicles Avoiding injury and damage to the testicle after testicular torsion is of the upmost importance. Many experts have suggested that the use of antioxidants can prevent further problems following a twisted testicle. A 2011 study from Gulf Medical College, Ajman, United Arab Emirates showed vitamin E, when it was administered before the testicles were twisted, provided significant protection against torsion and detorsion injury. A 2009 study from Dicle University, Diyarbakir, Turkey demonstrated that lycopene, an antioxidant, could help decrease oxidative stress- induced problems after testicular torsion, including issues surrounding fertility. 11. Magnetic Resonance Imaging Helps Identify Twisted Testicles A 2006 study from Kurashiki Central Hospital, Miwa, Kurashiki, Japan demonstrated the usefulness of magnetic resonance imaging (MRI) for patients presenting with the symptoms of twisted testicles. Thirty-nine patients who presented with twisted testicle symptoms underwent a magnetic resonance imaging scan and the diagnosis was made based on the images the scan turned up, not on any clinical history or examining the testicles. The MRO scan turned up the correct diagnosis in all of the cases of twisted testicle. 12. Undescended Testicles. Another possible issue for the little guys that could lead to testicular torsion is cryptorchidism, a complicated word for when the testis do not properly descend into the scrotum before birth. The National Institutes of Health assuages that while this is ¨fairly common¨in infants (occuring in about 4% of births), “in most cases the testicles descend by the time the child is 9 monhts old.¨ Cryporchidism happens because of the strength of a muscle reflex that retracts the testicles, and/or because of the small size of testicles before puberty. If the testicles do not descend by the age of 1, the condition should be given serious attention because undescended testicles have “an increased likelihood of developing cancer,” may cause infertility later in life, and could increase the risk of testicular torsion. If self- correction does not occur by the age of one, treatment options include hormone injections or surgery. 13. Testicular Tumor. In rare cases, testicular cancer can cause twisted testicles. Testicular cancer can occur in one or both testicles of men or young boys. This could appear as a lump or any other irregularity on the testicle, a feeling of heaviness in the scrotum, a dull ache in the groin, or general pain in the scrotum. And in addition to the many woes of cancer, Drs Erika Ringdahl and Lynn Teague with the University of Missouri-Colombia School of Medicine find that testicular cancer can also lead to testicular torsion. The Cleveland Clinic Foundation explains that while precise causes of testicular cancer are unknown, there are risk factors that could raise the odds of you or a loved one. Those between the ages of 15 and 40 are most vulnerable, and the condition is five times more likely in Caucasian men than for African American Men, and even less common in Asian-Americans. Unfortunately, risks also increase if there was an undescended testicle at birth (cryptorchidism, see above), or if someone in your family has suffered testicular cancer. But after all of this, there is good news! First of all, testicular cancer is rare and even when it does occur it is ¨highly treatable and usually curable¨ through surgery or radiation. Indeed, if the tumor is discovered before it has spread to the lymph nodes the cure rate is greater than 98%. Which is why the Cleveland Clinic Foundation stresses that “all men should be familiar with the size and feel of their testicles, so they can detect changes.¨ Might as well take advantage of the benefits of such a vulnerable design.
